My buddy has a Mazda5. He bought it used at a non mazda dealer with about 5000 kms.

He currently has 19,000 kms on it.  He has had to change his brakes 3 times (just pads). 

Has anyone else had any problems with the pads going so quickly?

wow lol.  I think he has a very heavy foot, changing brake pads every 4500 is rather extreme.  I'm still on the same brake pads that came with the car, 47000 km's now.  Are all brake pads wearing evenly and dying off at 5000km's?
I'm sure if there were something wrong with the calipers the techs would have said something though, so it might be due to his drive style.  Is he cutting through brake rotors as well?

I recall something about Mazda5 pads wearing out faster than usual when I worked at the dealer, try calling them.

They do wear quickly but you should still get about 30,000km out of the front pads.

The 5's pads will wear quicker than the 3's because they are designed for the 3.  All the extra weight in the 5 is certainly going to take it's toll on brake life.  HOWEVER, 5,000 K intervals indicates a problem.  Take it to the dealer and find out what is wrong.  If you don't get an answer go to a brake specialist, get them to diagnose the problem (might cost a half hour's labour) then take the report back to Mazda.  That should be covered by warranty.
